Fitness to Return to Work Letter Alberta
A fitness to return to work letter, sometimes called a 'fit note' or 'medical clearance,' is a formal document from a physician confirming that you are medically able to resume your job duties. In Alberta, many employers require this documentation before allowing an employee back on the job, particularly after absences of 3 or more days, or in safety-sensitive roles.
MedLetter provides same-day fitness assessments from CPSA-registered physicians, delivered as a signed PDF to your inbox. No clinic visit required. No video call. Just a straightforward online assessment and same-day documentation.
When Do You Need a Fitness to Return to Work Letter?
Most Alberta employers require a fitness letter after extended absences (typically 3+ days), for safety-sensitive positions (construction, oil and gas, transportation, healthcare), after surgery or hospitalization, after a mental health leave, or when returning from a medical leave of absence.
Even if your employer hasn't specifically asked for one, having a fitness clearance letter protects both you and your employer. It documents that a physician has confirmed you can safely perform your duties, reducing liability for both parties.
What Does a Fitness Letter Include?
Your fitness to return to work letter will include: confirmation that you were assessed by a CPSA-registered physician, a statement that you are medically fit to resume your job duties, the date you are cleared to return, any temporary restrictions or limitations (if applicable), the physician's name, signature, and CPSA registration number.
The letter does NOT include your diagnosis, treatment details, or any private medical information. Your employer only needs to know that you've been cleared, not why you were absent.
Full Clearance vs. Clearance with Restrictions
If you're fully recovered and can perform all aspects of your job, you'll receive a full clearance letter. If you're mostly recovered but have some temporary limitations, the physician can issue a clearance with noted restrictions.
For example: 'Cleared to return to work effective [date]. Temporary restriction: no lifting over 20 lbs for 2 weeks.' This allows you to return to work while protecting your recovery.

Safety-Sensitive Positions
If you work in a safety-sensitive role (oil and gas, construction, transportation, heavy machinery), your employer may have additional fitness requirements beyond a standard letter. In most cases, our physicians can provide the documentation needed. However, some safety-sensitive roles require specific occupational health assessments that must be done in person.
If your situation requires in-person assessment, we'll let you know and provide a full refund. We never issue documentation that isn't clinically appropriate.
Frequently Asked Questions
Is a 'fitness to return to work letter' the same as a 'return to work letter'?
Yes. Different employers use different terminology, but both confirm medical clearance to resume duties. Some call it a 'fit note,' 'medical clearance,' or 'fitness for duty letter.' They all serve the same purpose.
Can the letter include restrictions?
Yes. If you are fit to return but with temporary limitations (no heavy lifting, reduced hours, etc.), the physician will note these restrictions in the letter.
Do I need this for every absence?
Not necessarily. Most employers only require clearance after 3+ days of absence, for safety-sensitive positions, or after surgery/hospitalization. Check your employer's specific policy.
How quickly can I get the letter?
Most fitness letters are delivered within 1-4 hours of completing the intake form. Submit before noon for guaranteed same-day delivery.
What if I'm not fully recovered?
If you're not ready to return to full duties, the physician can issue a modified duties letter instead, specifying what you can and cannot do while recovering.
Fitness clearance letters are issued only when clinically appropriate after physician review. Some safety-sensitive positions may require in-person occupational health assessment. MedLetter does not provide WCB documentation.
