How to Register for Accommodations at SAIT
- Complete the Intake Form: Submit SAIT's online intake form through the Accessibility Services portal. This initiates your registration process.
- Receive FAF and Booking Link: After submitting the intake form, you'll receive an email with SAIT's Functional Assessment Form (FAF) in PDF format and a link to book an appointment with an Accessibility Advisor.
- Get Documentation Completed: Have the FAF filled out by a medical professional, OR submit existing medical documentation. Email to accessibility.services@sait.ca or bring to MC221.
- Attend Advisor Appointment: During your 1-hour appointment (in-person, phone, or online), review your documentation with the advisor. They'll create an accommodation memo outlining your approved accommodations.
Documentation Requirements
Mental Health: SAIT's Functional Assessment Form completed by a physician, OR a doctor's letter including diagnosis, learning barriers affecting academics, and suggested accommodations. Must describe how the condition creates barriers in SAIT's learning environment.
ADHD: Psychoeducational assessment report, physician's letter confirming ADHD with academic impact details, or completed Functional Assessment Form. Should address impact on both theoretical and practical program components.

Does SAIT accept online physician documentation?
Yes. SAIT accepts documentation from any qualified medical professional, whether the appointment was in-person or virtual. They accept their own Functional Assessment Form, a doctor's letter with learning barriers and suggested accommodations, or a psychoeducational assessment.
What is SAIT's Functional Assessment Form (FAF)?
The FAF is SAIT's standardized form that asks healthcare providers specific questions about your condition, functional limitations, and recommended accommodations. It's designed to give Accessibility Advisors the information they need to determine appropriate supports. You receive it after submitting the intake form.
Do I need to renew my SAIT accommodations each semester?
No. Since Fall 2022, your accommodation memo is valid for the duration of your program. However, you must share it with your instructors at the beginning of each semester. If your needs change, book an appointment with your Accessibility Advisor to update your memo.
Can I start the process before I have documentation?
Yes. You can complete the intake form and book an advisor appointment even without documentation ready. Your advisor can discuss your needs and guide you on what documentation to obtain. However, formal accommodations cannot be implemented until documentation is provided.
