List the employer requesting the note. If both need documentation, one note covers all concurrent employment for the same dates.
Can I get a note for a burn/cut from the kitchen?
Yes. Kitchen injuries that prevent safe food handling are legitimate reasons for absence. For serious burns or deep cuts, please also seek in-person medical treatment.
